Disability Benefits Questionnaire (DBQ)
The DBQ is a medical questionnaire that your private or VA doctor can complete on your behalf. It contains questions specific to your medical condition.
The DBQ is optional when submitting your claim, however, if properly done and accepted by the VA it could enhance your claim. If accepted by the VA, they will not require you to do a C&P exam. because completing the DBQ is what the C&P exam is for.

Note:
1. VA Healthcare Doctors are authorized to complete DBQs for veterans.
